What is trabeculectomy?

Trabeculectomy is a well-established glaucoma operation that creates a new drainage channel to release fluid and lower eye pressure. It has been performed successfully for many years and is often described as the gold standard for lowering pressure when a large reduction is required.

The operation works by making a small, guarded opening that allows fluid to drain from inside the eye to a space beneath the thin membrane covering the white of the eye, where it is gently absorbed. This forms a small reservoir, called a bleb, usually hidden under the upper eyelid. Trabeculectomy is generally used when drops or laser have not controlled the glaucoma sufficiently, or when the pressure needs to be lowered a great deal to protect sight that is already at risk. Because it creates a genuine new drainage route, it can achieve a bigger and more durable reduction in pressure than many other treatments. It is a more involved operation than laser or minimally invasive surgery, and it requires close follow-up while it settles, so your consultant will only recommend it when the benefits clearly outweigh the alternatives for your eyes. According to the NHS, this surgery is crucial for managing glaucoma effectively.

What trabeculectomy involves

Ms Brinda Shah forms a small drainage pathway under the eyelid so fluid can leave the eye in a controlled way. The operation is carried out in Yeovil, usually under local anaesthetic, so the eye is fully numbed and you do not feel pain. During surgery, a carefully guarded flap is created so that fluid drains at a controlled rate, and medicines are often used to help keep the new channel working well over time.

Trabeculectomy usually takes around forty-five minutes to an hour and is normally a day case, so you can go home the same day. The period after surgery is as important as the operation itself, because the new drainage channel needs to settle at the right level, and this is managed through regular reviews in the weeks that follow. Your consultant may adjust the healing with additional treatment during this time to fine-tune the pressure. Having a glaucoma specialist oversee both the surgery and the careful aftercare gives the best chance of a stable, lasting result, and you will be given clear guidance on drops and activity while the eye recovers.

Trabeculectomy benefits and considerations

Trabeculectomy can achieve a significant, lasting drop in eye pressure and protect your remaining vision. It involves a longer recovery and regular reviews, and carries surgical risks your consultant explains fully. Outcomes are never guaranteed.
